Pursuant to Part C of the Individuals with Disabilities Education Act (IDEA) as amended in 2004, the Illinois Department of Human Services has posted the Federal Fiscal Year (FFY) 2012 Annual State Application Under Part C of IDEA. Part C of IDEA addresses services for children birth to 3 years of age with developmental disabilities or delays and their families. The application will be posted at least 60 days prior to being submitted. You may view the application by going towww.dhs.state.il.us/ei then go to Reports/Applications. Copies for review are available at all 25 Child and Family Connections offices. For Illinois residents, you may locate the CFC office nearest you by calling 800/323-4769.
Public comment will be taken on the application for 30 days. Comments will be accepted beginning February 9, 2012. All comments must be received in writing by close of day April 9, 2012. Please send written comments to the following address.

In addition, before a State adopts a new or revised policy that is not contained in, but required to be part of, Section II.A of the FFY 2012 Annual State Application Under Part C of IDEA, it must post the new or revised policy for at least 60 days, with an opportunity for public comment for 30 days. New or revised policy can be found in “Section II.A.3.a Addendum Proposal,” which is an attachment to the 2012 Annual State Application Under Part C of IDEA. Please note the above instructions on how to access the application. Comments will be accepted beginning February 9, 2012. All comments must be received in writing by close of April 9, 2012. Public hearings must be held on this new or revised policy during the 60-day publication period (with notice of the hearings provided at least 30 days prior to the hearings). The dates and location of the public hearings are listed below. Each person presenting oral testimony is asked to provide a written copy of his/her testimony at the time the testimony is presented.
